Membership Application An application for individual membership or renewal will be required to pay HK $ 20 annual fee (payable to payable to "Hong Kong China Bodybuilding And Fitness Association") Individual Member: Download The Entry Form for Individual Membership Renewal Application: Download The Renewal Membership Form for Individual Membership Individual Membership Application Procedures ● Application forms can be downloaded at our website (www.hkbba.org) or to HKBBA office. ● Applicants complete the application form, together with recent photos and your dues to be returned to HKBBA. ● Applicants may visit Hong Kong China Bodybuilding Association office, fax or by mail to submit a completed application form and relevant documents Please mark the envelope "Individual Membership Application". Club Membership Application Procedures Admin. Fee: HKD$ 400 Valid period: 1 year ● Application forms can be downloaded at our website (www.hkbba.org) or to HKBBA office. ● Fill in the application form attached with a copy of the Business Registration Certificate and deliver the cheque (Name of payee: HONG KONG CHINA BODYBUILDING AND FITNESS ASSOCIATION) by mail to Hong Kong China Bodybuilding And Fitness Association ● Applicants may visit Hong Kong China Bodybuilding Association office, fax or by mail to submit a completed application form and relevant documents Please mark the envelope "Club Membership Application". ● We will mail the club membership certificate to your club after our approval. Download The Club Membership Application Form  Payment method We will accept cash or check payment method ● Except in special circumstances and legal requirements, all expenses paid shall not be liable. ● Tuition and places non-transferable. ● Applicant will receive a receipt at payment of a fee. The mistakes would be lost due to postal receipt shall not be responsibl. Cash Please do not send cash. This would be lost due to postal errors in payments we shall not be responsible. Check Payable to "Hong Kong China Bodybuilding And Fitness Association" Application notes Enrollment procedures The degree awarded and professional courses, please use the designated application form. Application Forms can be downloaded (www.hkbba.org) or to the HKCBBA Office. ● Completed application forms together with the required photos, fees and required documents will be returned. ● Applicants can visit Hong Kong China Bodybuilding Association office, fax or by mail to submit application forms and related documents. Please specify course name on the envelope. Note application form ● In block letters. ● Make sure that the information filled in application forms are clear and correct. Incomplete application forms will not be accepted. (Remark: Please note the e-mail address and contact number for the class time students will be notified and special event) ● Make sure to sign the application form. Notification ● Successful applicants will receive before the start of the course will be issued by the admission notice. If not received in a week before commencement notice, please contact us. ● If the applicant inquiries, contact the program staff.
